Some manufacturers choose to have the bottom blade moving forward and some backward. This is in spite of the fact that they each turn the main rotor blades the same way. What difference does it make?

The rotational direct of the tail-rotor has more to do with the supplier of the engines and transmission than anything in aerodynamics...Whether it's clock-wise or anti-clockwise really doesn't matter.
The way the transmission operates (ie:direction of the input quill and output quills) will determine which way the tail-rotor driveshaft turns and this will, in turn, determine the direction of the tail rotor...
With respect to the tail-rotor blades advancing into the downwash or with the downwash....most tail-rotors sit above the tip-path plane of the main rotor.
